[X3D-Public] X3D Schema, DTD corrections: meta tag name attribute is optional to match HTML

- meta tag name attribute is optional to match HTML, since http-equiv attribute might be used instead

New rules were also added to X3D Schematron to warn if these attributes
(name and http-equiv) are either both missing or both present.
