[x3d-public] X3Dv4 mantis issue resolutions

Dick and I discussed issue resolution process and applied fixes to four small issues today.  Issues that are fixed and committed to specification HTML source on github are marked "resolved" pending final group review of all completed issues.

Plenty more "minor" issues to go (with thanks to Roy Walmsley for strict review) but our mantis/github mechanics are improving.
